1/ Do you know what a mixing study is? Let's take you through some important facts. A short🧵
2/ A mixing study involves mixing patient plasma with normal plasma. It helps in the first stage of investigating a prolonged APTT 👇
If you have an unexplained and isolated APTT and no heparin, think of haemophilia. Mixing studies can help in the diagnostic pathway.
3/ However, be careful
🔴Most inhibitors detected in acquired haemophilia A (AHA) are heat & time-dependent. Hence mixing studies require incubation for 2 hours.
4/🔴In AHA, mixing studies tested immediately are likely similar to those seen in patients with a single factor deficiency & no inhibitor.
🔴Mixing studies are also poorly stan­dard­ised, limiting the comparison of results between laboratories.
5/ Key message
Further inves­ti­ga­tion is always required, and fac­tor activ­ity assays should be performed in par­al­lel with mixing studies to facil­i­tate early diag­no­sis, especially in acquired and congenital haemophilia.
