This is what seems to have happened:
1. Freight train comes first and is shunted to common loop track via switch 17A.
2. 17A should have switched back to UP Main for Coromandel Express. It didn’t. Train comes at 128kmph, derails at 17A.

Track which it was never supposed to be on. Some coaches fall on DN Main where Bengaluru-Howrah express is coming.

My railway map has Howrah and Madras switched, apologies. This is the way the map should be. 17A point was the problem, not 20B. Also Yesvantpur express already in station as Coromandel enters (h/t @DeepakaaR)
