🛑Adjustment of #insulin dose in #DM🛑
ضبط جرعة الانسولين يعتبر تحدي وخصوصاً في بداية صرفه لعدة أسباب قد تكون متعلقة بالمريض ، ولكن هذه هي البدايات فقط ف ثق بطبيبك المعالج ودعه يرشدك لضبط المستوى في نطاقه الصحيح
يلا نشوف كيف نتعامل مع الحالات:

طيب كيف نضبط وحدات الانسولين؟
١- نتكشف تقلّب مستوى السكر (انخفاض او ارتفاع) خلال اليوم
٢- متى حدث التقلّب بمستوى السكر تحديدًا
٣- سبب حدوث هذا التقلّب
(طبعاً النقطة ١ و ٢ يكون بمساعدة المريض بحيث يعطينا قراءاته خلال اليوم)

راح افصّل بأول حالة فقط مع مثال والحالات اللي تليها بيكون حلها على غرار اول حالة

Pt is known case of DM type 1 on rapid and long acting insulin. He did gluco check before breakfast and showed a reading of hyperglycemia
نستذكر النقاط الثلاثة (1- نكتشف ، 2- متى ، 3- السبب)
1- hyperglycemia
2- Fasting
3- Somogyi or dawn phenomenon?

الحالة الاولى عباره عن fasting hyperglycemia وسببها امّا not enough basal insulin (dawn phenomena) or insulin overdosing (somogyi effect)
How to treat?
If dawn phenomena > ⬆️ Basal insulin
If somogyi effect > ⬇️ insulin

Cause: ⬇️ rapid acting insulin at breakfast
How to treat?
⬆️ rapid acting insulin at breakfast

Predinner hyperglycemia
Cause: ⬇️ rapid acting insulin at lunch
How to treat?
⬆️ rapid acting insulin at lunch

Bedtime hyperglycemia
Cause: ⬇️ rapid acting insulin at dinner
How to treat?
⬆️ rapid acting insulin at dinner

Cause: ⬆️ rapid acting insulin at breakfast
