What is a Cognitive Forcing Strategy?
A cognitive forcing strategy is a "forced mental check that occurs during the diagnostic process for the explicit purpose of optimizing it."
Often in clinical medicine we make preliminary diagnoses -- CKD, pulmonary HTN, sepsis, nephrotic synd, hyperthyroidism, and more!
Yet, these often represent only the beginning of the diagnostic journey, and should prompt further evaluation to identify an underlying etiology.

Here is an example of reaching an endpoint diagnosis of cardiac amyloidosis, a diagnosis that is missed and sometimes chalked up only to "HFpEF".

In sum, endpoint diagnoses are final etiological explanations for other, more proximal Dxs.
We should always ask the ? whether the Dx we have reached is the final Dx, or simply a preliminary Dx that needs further elucidation.
A simple concept but very important!
