أول شيء بنأخذ مقدمة بسيطة عن HFNC
HFNC مقارنة بـ conventional o2 device
راح يحسن لي الـ Oxygenation
ككيف ؟
وله عدة advantages:
1-Reduced anatomical dead space
2-PEEP
3-Constant FIO2
4-Good humidification.

Indication:
1-Acute Hypoxemic Respiratory
Failure PO2/FiO2 ratio < 300 mmHg
2-Post-Extubation (Patients at low risk
for Re-intubation)
3-Pre-Oxygenation Prior to Intubation
4-Cardiogenic Pulmonary Edema
4-Patients with do-not-intubate(DNI)
status and respiratory failure.

Failure PO2/FiO2 ratio < 300 mmHg
2-Post-Extubation (Patients at low risk
for Re-intubation)
3-Pre-Oxygenation Prior to Intubation

Contraindications:
1-Low level of consciousness with
GCS < 9.
2-Abnormalities or surgery of the
face, nose, or airway that preclude

cannula.

ROX Index can help suggest which patients will fail HFNC and need to progress to intubation for further ventilatory support.
Setting HFNC :
•Fio2 ( 21% - 100% ).
•Flow (30 lpm - 60 lpm ).
•Temperature ( 31 ° -37 °).
(lower temperature usually leads more comfort when high flow is used).

متى وليش أرفع الـ
1-Respiratory rate fails to improve.

اول شيء قلناه ان HFNC
عنده advantages ومن اهمها ؟
انه يعطينا Peep وبيعطينا التأثير بشكل كبير لما يكون فم المريض مغلق.
ف دايم نقول للمريض انه وقت HFNC يتنفس وفمه مغلق والحين بنفهم الفكرة ليش.

Every increase of 10 L/minute of flow yields approximately 0.7 cm H2O of airway pressure when the mouth is closed and 0.35 cm of H2O when the mouth is open.
لما يتحسن وضع المريض من ناحية التنفس والاكسجين والاشعه واقدر اني اسوي weaning للمريض ف ابداء فيه
طبعاً لِكل مستشفى بروتكول معين الى HFNC ولكن تقريباً انها متشابهه.

1.Start by weaning FIO2 in 5% - 10% increments until <35%
2. Next, wean flow rate by 5-10 L/min every 2 to 4 hours as tolerated
3.Switching to Conventional O2 therapy.
