You hear COVID vaccines are highly effective, & protection will last years.
But you also hear people are getting COVID even after 2 doses of vaccines, & some have died.
What gives? Here are 4 reasons why excellent vaccines sometimes appear not to work:

1. Vaccine efficacy is not 100%: Remember that even the best COVID vaccines are only ~95% effective against infection. Some are only 70-80% effective Which means with large numbers of infections, you are going to see some people in whom the vaccine does not work.
2. Immunosuppression: Some people who are receiving cancer chemotherapy, or taking immunosuppressive drugs for autoimmune conditions, or have an underlying immunosuppressive condition, or some elderly people may not mount an adequate neutralizing antibody response to the vaccine.
3. High viral dose exposure: It is quite likely high viral dose exposures can overcome the immune system. For example, sustained or repeated maskless exposure to someone who is infectious poorly ventilated indoor setting, or in healthcare workers in crisis mode without proper PPE
4. Vaccine evasive variants: A variant that has mutated itself enough to evade the immune response produced by a vaccine. Already we know that VOCs, especially delta variant are capable of some vaccine evasion. It is possible new variants that evade vaccines can emerge.
The current vaccines are outstanding but not perfect. They give excellent protection against severe COVID. They save lives.
Do not be worried about reports of vaccine failures which may occur due to one or more of the reasons I listed. They are the exception.
